I find CAD is most useful if you need to design something in more 3D space, or if you’re designing something for a larger production. If I just need to make one of these, it’s easier to go straight from a technical drawing to manufacturing, and just tweak the parts to make it work as you go. If the mechanism requires a gear, it may be helpful to use CAD, but there are tools that will cut gears of different sizes anyway so it’s not necessary if you have those already. For replications, they would likely avoid engraving too much and would make some small molds to reproduce the components. The internal mechanisms could be laser cut or stamped, and you would need a skilled person to assemble the final piece.
